CHESAPEAKE -- A 13News Now Investigation has uncovered a string of safety violations tied to the construction company whose worker was killed Wednesday in a construction accident in Virginia Beach.

Police say 49-year-old Antonio Reid, of Chesapeake, died after being pinned under an excavator.

Reid worked for A&W Contractors, Inc, which is based in Chesapeake.

Records obtained from the Federal Occupational Safety & Health Administration show the company has been inspected by safety regulators ten times since 2003. The records show A&W Contractors was cited as a result of half of those visits.

The company's most recent violation came in April 2013, when inspectors cited the company for two serious safety violations: failing to use proper safety equipment to stabilize excavated holes and failing to use proper protective systems.

A&W Contractors was cited two other times in the past decade for failing to use proper safety equipment to stabilize excavated holes, once in July 2007 and again in July 2009.

OSHA records show the company was fined for all incidents and the problems were eventually corrected.

A woman who answered the phone at A&W Contractors on Thursday morning said the company did not wish to comment.

Both Virginia Beach Police and the Virginia Department of Labor and Industry are investigating the accident.
